Entries should demonstrate how the partnership has facilitated advancements in healthcare delivery, patient care, medical education, or healthcare technology. Submissions should highlight tangible outcomes, such as improved patient outcomes, or innovative healthcare solutions developed through collaboration Entry is by a 5-page A4 PDF submission; the judges will be looking for a clear plan and evidence that it is delivering on core objectives, paying particular attention to the criteria listed below.

• Team Background: Please outline the key members of the transformation team. Highlight the team's composition, roles and responsibilities, along with any unique attributes or qualifications that set your team apart.
• Project Overview: Summarise the digital transformation project(s) which the team worked on, including the objectives, budget, timeline, and all other key info .
• Teamwork: Outline any project challenges/obstacles faced and how the team demonstrated collaboration, creativity and flexibility to overcome them.
• Contributions & Achievements: Describe the contributions and the impact of the team on this project including innovations introduced, outstanding achievements, and benefits of the work carried out. Present measurable data that underscore the team's impact.
