Once you have created your PDF submission and you have all items in the Items Checklist, submit your entry via the Online Entry Form. CLICK HERE.

This award is open to both individuals and institutions in Ireland who have demonstrated exceptional commitment to improving the quality of life for patients facing serious illness or nearing the end of life.

Entry is by a 5-page A4 PDF submission; the judges will be looking for a clear plan and evidence that it is delivering on core objectives, paying attention to the criteria listed below.

  • Clinical Excellence: Please provide an outline of exceptional clinical skills in providing pain and symptom management, ensuring the highest quality of care for patients.
  • Innovative Practices: Please outline how the steps you take to implement innovative approaches and practices that enhanced the quality of life and care for palliative care delivery, contributing to advancements in symptom management, pain relief, and overall patient comfort.
  • Patient and Family-Centred Care: How did you provide a compassionate and holistic care that considers the physical, emotional, and spiritual needs of patients and their families?
  • Outcomes and Impact: Demonstrate measurable outcomes and impact in improving the quality of life for palliative care patients and their families.
  • Impact and Sustainability: Provide an outline of a measurable impact and sustainability of palliative care initiatives, demonstrating long-term benefits for patients, families, and the wider healthcare system, and inspiring others to adopt similar practices.
  • Education and Advocacy: Provide Provides an outline of initiatives to raise awareness about palliative care, dispel myths and misconceptions, and promote access to high-quality palliative care services within the community and healthcare system.
